In our effort to make your first visit to our office as seamless as possible you were directed to this website by a member of our office staff to save you time in the still necessary gathering of information. From your computer at your home or office it will take only a few minutes to download the appropriate forms and print them.

We direct you to select the following forms, the “Patient Information”, the “Medical History” and the “Dental History”. If the appointment is for a younger member of the family please download the “Dependant Information” and be sure to include the medical and dental history forms for each dependant.

By pre-printing the forms you will now be able to fill in all the information at your leisure which we hope saves you time in an already busy schedule and there’s no pressure “to get it done” in the office.
